Employees if Drayer Physical Therapy Institute® (DPTI) are provided a wide range of health and welfare benefits. A number of the programs (such as Social Security, workers’ compensation, state disability and unemployment compensation) cover all employees in the manner prescribed by law. Additionally, DPTI offers an Employee Assistance Program to all employees and their families

Health

Medical & Vision

The company offers two standard medical plans that cover major medical, hospitalization, and a prescription drug and vision program. The medical plans are offered to all full-time employees who work 32 or more hours per week. Both the company and employee share the cost of coverage. The employee may elect coverage for himself/herself and family. If an employee’s spouse is eligible for medical coverage through their own employer, but chooses to be covered by the DPTI medical plan, the employee will incur a per pay surcharge to keep their spouse on DPTI’s medical plan. Medical coverage will go into effect the first of the month following 30 days of full-time employment.

Dental

The employee and their family members may also elect dental coverage. The dental plan is offered to all full-time employees who work 32 or more hours per week. Dental premium costs are also shared by the employee and by the company. Dental coverage will go into effect the first of the month following 30 days of full-time employment.

Paid Time Off (PTO)

DPTI believes it is necessary and healthy for employees to schedule time away from work each year for rest, recreation and relaxation. To provide for such absence we provide paid time off for all full-time (32 or more work hours each week) employees. Employees are encouraged to use their full allotment of paid time off each year.

DPTI employs a Paid Time Off system (PTO), which incorporates vacation and sick time. Employees accrue (earn) annual paid time off according to the following schedule:

Years of employment are measured from the first day of full-time employment with DPTI.

Retirement Savings Plan (401K)

DPTI offers a 401(k) Employee Retirement Plan. The Plan is defined as a voluntary, employee contribution long-term savings plan. The Plan enables employees to save money via payroll deductions on a pre-tax basis and invest into a variety of stocks and bonds offered by the Plan.

Eligibility Requirements:

  • Full-time employment of 32 hours or more per week
  • At least 21 years old
  • All employees that work 1,000 hours or more per year

Employees, who meet the above criteria, will be automatically enrolled in the plan the first of the month after date of full-time hire.

Employees may elect to defer between 1% and 80% of their salary (IRS annual limits apply) on a pre-tax basis. Employees have the option to make changes to their deferral percentage or investment selection any time.?Plan rollovers, loans and withdrawals may be permitted.

Charity Day

DPTI is committed to leading the way to good health and additionally we are committed to supporting our employees as they actively participate in local charities. One way we would like to show our support is by allowing employees one day per year that can be used to participate in a charitable event. One paid charity day per year is granted to full-time employees who work 32 hours or more per week. DPTI will compensate up to eight hours of pay per year for the day of charity.

Additional Insurances (STD, LTD, LIFE, & AD&D)

Short Term Disability

Short Term Disability (STD) income is provided to all full-time employees who work 32 hours or more per week and who are absent from work for extended periods of illness or disability. DPTI pays 100% of premium for each employee. Total disability is defined as the inability of an employee to perform the material duties of his/her occupation as a result of sickness, injury or pregnancy.
STD is designed to provide a partial salary payment in the amount of 60% of weekly base earnings up to a maximum of $1,000 per week. The maximum payment period is 25 weeks.

Long Term Disability

Long Term Disability (LTD) income is provided to all full-time employees who work 32 hours or more per week and who are absent from work for extensive periods of illness or disability. The LTD benefit provides 60% of monthly earnings to a maximum benefit of $5000 per month. LTD begins after 180 days of disability and the maximum benefit period is determined by the employee’s age when disability follows. Premiums for LTD are paid 100% by DPTI.

Life Insurance and Accidental Death and Dismemberment Insurance

DPTI provides all full-time employees who work 32 hours or more per week, a group term life insurance. The life insurance is 1.0 times the annual base salary to maximum life insurance coverage of $100,000. DPTI pays 100% of the premium for group term life for each full-time employee.

The company also provides accidental death and dismemberment insurance (AD&D). The AD&D insurance is 1.0 times annual base salary to a maximum insurance coverage of $100,000. DPTI pays 100% of the premium for AD&D for each full-time employee.

In addition to basic life insurance and AD&D, DPTI also offers supplemental life insurance, which allows employees to purchase up to an additional 2.0 times their annual base salary to a maximum coverage of $300,000. Cost is paid by the employee and is based on employee’s age and salary.

The company also offers dependent life insurance for the employee’s spouse and children at an additional fee paid by the employee.
